Post-Acute Care

Premier provides care for those patients requiring rehabilitation for their transition home after an acute hospital stay. This includes physical therapy, occupational therapy, speech therapy, nutritional services and education.


Our interdisciplinary team of pulmonologists, respiratory therapists and nurses work together to develop maximize each patient’s ability. We monitor and adjust oxygen therapy, utilize medication and nebulizer treatments as well as exercise tolerance to assist each patient to meet their short, and long term goals.


We provide cardiac rehabilitation, which includes a team of physicians, nurses and therapists, to develop individualized programs tailored to meet the patient’s specific needs to maximize the patient’s abilities.


Our team of specialists provide medical and nutritional support to help resident’s control their blood glucose levels and maintain a healthy lifestyle.
